Our case management staff will begin assisting you upon admission to the hospital to facilitate your discharge. The nurses and social workers of case management can provide information, referral counseling, crisis intervention, case management and discharge planning services to inpatients.
Based on physician orders, we arrange for your home healthcare, hospice, medical equipment, legal aid, housing, food and protective services.
We strive to orchestrate safe, effective transitions through the healthcare continuum. We work collaboratively with all members of the multidisciplinary team.
We strongly believe the patient and patient's family are an essential part of that team.
We are located on the 3rd floor, next to the Medical/Surgical Department.
